Inspector Notes

An unannounced monitoring inspection was conducted on 2/7/2023 with center director. The inspection began at approximately 12:30pm and concluded at 2:00pm. There were 54 children present, ranging in ages from 4 years to 5 years old, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the center did not ensure that electrical outlets had protective covers. Evidence:
1. During the inspection, the inspector observed five uncovered outlets in the gym. The Pre-K students were observed napping in the gym.
Plan of Correction: Outlet covers will be covered right away.
Standard 8VAC20-780-60-A
Based on record review, children's records were incomplete.
Evidence:
1. Child #1?s record (start date 9/12/2022), did not contain documentation names of persons authorized to pick up the child, and missing the child's doctor's name, address and phone number.
Plan of Correction: Will reach out to the family of child #1 to receive pediatrician as well as pick up information.
