Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on July 26, 2024. The on-site inspection began at 8:50 AM and ended at 10:40 AM. The inspector reviewed compliance in the areas listed above. There were 47 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included a tour of facility, document view, interviews, and observation.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on observation, the center did not ensure that nonprescription medications shall be in the original container with the direction label attached.

Evidence:
Child A has Allergy Relief medication on site. The medication was in a plastic bag with the child's name on it and not in the original packaging.
Plan of Correction: Original med box will be placed with medication.

Based on record review, the center did not ensure that written the record of children's serious and minor injuries was complete.

Evidence:
2 of 5 injury reports to did not contain staff and parent signatures or two staff signatures.
Plan of Correction: Signatures will be collected from parents.
