Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/30/2024. The on-site inspection began at 11:40 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 9. The inspector reviewed 5 children's records, 1 provider record, and 1 house member record on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The license and the findings from the most recent inspection were not posted in a conspicuous place on the premises.

Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parent's signed acknowledgment of the review. 4 out of the 5 children?s records reviewed did not have a parent?s signature of an annual review being done within the last year.
