Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A renewal inspection was initiated on Wednesday, September 8, 2021 and concluded on Thursday, September 9, 2021. The provider was contacted by telephone and a remote inspection was conducted. There were 2 children present with 1 staff supervising. A total of 2 children records and 1 caregiver records were reviewed. Based on review of the caregiver's record and interview, the provider did not ensure to complete the required annual training.

Evidence: In Caregiver #1's record, there were a total of 4 out the16 annual training hours verified during this inspection. Interview with provider confirmed the 4 hours of completion for the annual training.
Plan of Correction: The provider will ensure to participate and obtain documentation of all annual trainings for future inspections and remain in compliance with licensing standards.

Based on review of providers record and interview, the repeat check for search of central registry finding was not found.

Evidence: In Caregiver#1's record, there were no findings found for an updated search of central registry. The last dated search was March 19, 2015. Interview with caregiver revealed a search was request but there were no findings available for review during this inspection.
Plan of Correction: The provider already submitted the document to request search and will follow up with department . The provider will ensure to include the repeat check for the central registry in the record for further review.
