Inspector Notes

A monitoring inspection was initiated and concluded on Thursday, May 27, 2022 in person at the family day home. There were 4 children present with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 4 child records and 1 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of children's record and interview, the provider did not ensure to obtain documentation of a physical examination within 30 days after the first day of attendance.

Evidence: During the inspection on May 26, 2022, in Child#1's and Child#2's record (start date 8/30/2021, there was no documentation of physical examination found. Interview with provider confirmed the documentation was not available during this inspection.
Plan of Correction: The provider will ensure to obtain all missing physical documentation and include in children's records. Provider will ensure all physical are obtain within 30 days.

Based on review of children's record and interview, the provider did not ensure to obtain in 2 of 4 records all information for children's record.

Evidence: During the inspection on May 26, 2022, in Child#3's record (start 4/13/22), the emergency contact address was missing. Interview with provider confirmed the information available for review in the record.
Plan of Correction: The provider will ensure to obtain all information for children's record is complete.
