Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on March 9, 2022 and concluded on March 14, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were 39 children present, ranging in ages from 5 to 11, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, record review and staff interviews, the licensee did not ensure that in each grouping of children there was at least one staff member who meets the qualifications of a program leader.

Evidence: At approximately 8:20 am during the before school program there were only 2 staff, (Staff #3 and Staff #5) supervising 16 children in the cafeteria and there was not a staff member who met the qualifications of a program leader.
Plan of Correction: The site supervisor will ensure that children are being supervised by staff that meet the program leader qualifications when grouping for activities.
