Inspector Notes

An unannounced renewal inspection was conducted on 03/20/2024 and 03/21/2024. There were 5 children present directly supervised by 7 staff. The physical plant, 5 staff records, 4 children?s records, medications, evacuation drills, and emergency supplies were inspected. Children were observed participating in free play. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 1 of 4 children's records did not contain a physical examination and immunizations signed by a physician, his designee, or an official of a local health department.

Evidence:
1. The immunization document contained in the record for Child #4 (start date 8/23/2023) was not signed by a physician, his designee, or an official of a local health department.
2. The physical examination in the record for Child #4 was not signed by a physician, his designee, or an official of a local health department.
Plan of Correction: A signed physical was obtained on 10/11/2023. The center will request a copy of immunizations signed by a physician.
