An announced monitoring inspection was conducted on 6/3/2024. A review of staff personnel files was conducted by inspectors, Keesha Minor, Jessica Willis, Sarah Zirzow and Mahrukh Aziz at the Campagna office. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not obtain repeat background checks every five years as required for 1 of 7 staff reviewed.
