Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on 12/16/2024, as a part of the licensure period. The on-site inspection began at 1:05pm and ended at 2:15pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Documentation must be maintained of emergency evacuation, shelter in place, and lockdown drills and include all required information. Documentation of emergency drills did not contain documentation of the person conducting the drill, number of staff members participating, and the method of notification.
Plan of Correction: Center will discuss forms used for documenting emergency drills with upper management.
