Inspector Notes

A mandated monitoring inspection was conducted on May 8, 2023 from 9:10 A.M.-11:30 P.M. There were 49 children present, ranging in ages from five months to five years of age, with eleven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and nine staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

REPEAT
Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location.

Evidence:
1. In the Rainbow Classroom there was a spray bottle of bleach water sitting on the counter next to the diaper changing table, and a spray bottle of Febreze sitting on the shelve next to the microwave.
2. In the Pre-K restroom located in the classroom, there were two spray cans of Lysol Disinfectant Spray in a pink container located in bathtub.
3. The manufacturer's label stated "Keep Out of Reach of Children", and "Caution."
Plan of Correction: Staff explained that the babies could not reach the table. Administration explained that is standard and it must be kept locked. Locked filing cabinets were purchased for the Pre-K room.

Based on observation, the center failed to ensure that the diapering surface shall only be used for diapering or cleaning children.

Evidence:
On 05/08/2023, in the Circus Classroom, there was a small crochet blanket, Mickey Mouse stuffed toy, and a pack of diapers setting on top of the diapering surface.
Plan of Correction: Administration spoke to staff and reminded them that changing tables should not have anything placed on them.
