Inspector Notes

A non-mandated focused inspection was conducted on 10/30/2023 from 10:34 A.M.-12:15 P.M. There were 52 children present, ranging in ages from four months to five years of age, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, and special care and emergencies A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of children records, the center failed to ensure that all required information was in the children's records.

Evidence:
1. Child #2 and Child #3's records did not contain the name, home address, and home phone number of the parent/guardian who has custody of the child.
2. Child #3's was missing the name, address, and phone number of one of the two required designated people to call in an emergency if the parent cannot be located.
Plan of Correction: Parents corrected at pick-up.
