Inspector Notes

An unannounced, on-site renewal inspection was initiated on 03/31/2026 and completed on 04/06/2026. The on-site inspection began at 10:20 AM and ended at 12:15 PM. The inspector reviewed compliance in the areas listed above. There were 61 children present and 14 staff. The inspector reviewed 6 children?s records and 6 staff records onsite. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 04/13/2026. A POC submitted after this date will not appear on the public website.

The center must request a search of the central registry prior to the employee's first day of employment.

The central registry was not sent prior to hire for Staff # 2, 3 and 5. The central registry was sent 3 weeks after the hire date.

Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall.

The piece of climbing equipment next to the fence did not have the 6 feet of fall zone required.
