Inspector Notes

A monitoring inspection was initiated and concluded on 2/1/2023. There were 31 children present with 4 staff supervising
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
A total of 8 staff records were reviewed. The inspection started at 12:15 pm and concluded at 2:55 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the religious exempt program did not ensure that the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years were obtained for each employee.

Evidence:
Eight staff records were reviewed.
The record for staff #4, whose employment began 8/17/2020, contained a Signed Sworn Disclosure Statement that indicated that staff #4 resided in Massachusetts within 5 years of her employment.
The file for staff #4 did not contain the results of a child abuse and neglect registry or equivalent registry from Massachusetts.
The office manager confirmed that this was an oversight and that this documentation had not been obtained for staff #4.
Plan of Correction: The results of the child abuse and neglect registry or equivalent registry in Massachusetts, for staff #4, was requested and obtained by the office manager and a copy sent to the inspector on 2/7/2023.
