Inspector Notes

An unannounced monitoring inspection was conducted on Thursday, June 27, 2024, to determine the provider's compliance with licensing standards. The inspector was on site from 9:25am to approximately 10:30am. During the inspection, there were a total of six children in care,
totaling 12 points, in the direct care of one caregiver. Upon arrival, the children were observed in their designated area of the family day home. The children were playing and interacting with one another and the provider. Later, the children were observed during a group activity. All areas used for childcare were inspected. The family day home has age and stage appropriate materials and supplies. The provider was observed having positive interactions with the children in care. The first aid kit and non-emergency supplies were inspected and found complete. Per the provider, transportation is not provided and medication is not administered at this time. One caregiver's records and five children's records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-800-100-A
Based on a review of five children's records and interviews, the provider did not obtain documentation of one child's physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: The record of Child #2, date of attendance 04/01/24, did not contain documentation of a physical examination. During interview, Caregiver #1 reported the physical examination could not be located at the time of the inspection.
Plan of Correction: Parent submitted the immunizations and physical examination on 07/01/24.
Standard 8VAC20-800-90-A
Based on a review of five children's records and interviews, the provider did not obtain documentation that one child has been adequately immunized according to the requirements of ?32.1-46 A of the Code of Virginia and applicable State Board of Health regulations prior to attendance.

Evidence: The record of Child #2, date of attendance 04/01/24, did not contain documentation of immunizations. During interview, Caregiver #1 reported the immunizations could not be located at the time of the inspection.
Plan of Correction: Parent submitted the immunizations and physical examination on 07/01/24.
