Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and completed on 4/6/21. The director was contacted by telephone to initiate the inspection. There were 22 children and 7 staff present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 2 staff records, and other center documents submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. For any questions related to this inspection, please contact me at 571-835-4718 or mahrukh.aziz@dss.virginia.gov.

Violations

1
Standard 63.2(17)-1720.1-A
Based on records review, the center did not obtain the results of a repeat Virginia central registry finding for an employee within 5 years.
Evidence:
1. Staff # 1's VA central registry findings that were obtained on 10/22/15 expired on 10/22/20.
2. Staff # 1's most recent VA central registry was dated 3/15/21 showing that it was not obtained within 5 years of the previous central registry check.
Plan of Correction: Moving forward the center director will be completing all central registry checks to ensure that the results are obtained on time. The director will implement a system that includes sending the forms to the agency, and conducting follow ups if results are not obtained on time.
