During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 1/23/25. The on-site inspection began at 11:40am and ended at 12:50pm. The inspector reviewed compliance in the areas listed above. There were four children present and one caregiver; the point total was eight. The inspector reviewed three children?s records and four caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/7/25. A POC submitted after this date will not appear on the public website.
Violations
3Caregiver A did not complete 16 hours of annual training.
Two electrical outlets were observed uncovered in the daycare area. The outlets were within children?s reach and directly above a sleeping child.
The record for Child A was missing a parent?s work address, both parents work phone numbers, signed policies for medication and liability insurance, documentation of obtaining proof of the child?s identity from the parent, and documentation of annual review. The child has been enrolled for over 16 months. The record for Child C was missing documentation of obtaining proof of the child?s identity from the parent. Child C has been enrolled for over seven weeks.
