Inspector Notes

A subsidy health and safety (SHSI) inspection was initiated on September 19, 2023 and concluded on September 22, 2023. There were 15 children present with 4 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-600-B
Based on record revies and staff interviews, the vendor did not ensure staff who work directly with children shall have completed Virginia Preservice Training.
Evidence: The record for Staff #3 (date of hire 2-16-2018) did not include documentation of completing the required training.
Plan of Correction: The center will ensure all staff have completed the VA preservice training within 90 days of hire. The preservice training will become a part of the initial onboarding and training process.

Based record review and staff interviews, the vendor did not ensure staff who work directly with children shall have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence: The following records did not include CPR documentation as required
1. Staff #3?s CPR certification expired April 24, 2023.
2. Staff #1 (date of hire 5-9-2023) did not include documentation of obtaining of CPR training.
Plan of Correction: The center will ensure all new staff have CPR/1st aid ordination within 30 days of hire and is signed up for the CPR/1st aid class within 90 days of hire.
