Inspector Notes

An unannounced inspection was initiated on 04/16/2025 and completed on 05/05/2025, in response to a complaint received by the licensing office on 3/27/2025 relating to allegations of programs. The inspector reviewed 3 staff records on-site. This inspection included document review,tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to special care provision and emergencies and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 5/19/2025. A POC submitted after this date will not appear on the public website.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include any future action to prevent recurrence of the injury;and staff and parent signatures or two staff signatures.

There was not two signatures on the injury report provided and there was no future action to prevent occurence on injury report #1.
