Inspector Notes

An unannounced code compliance inspection was conducted on 5/2/2024 from 10:10AM to 12:15PM. There were 45 children present, with 5 supervising staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision and procedures. A total of 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard 22.1-289.035-B-2
Based on record review, a fingerprint-based criminal history record check was not obtained prior to the 1st day of employment for 2 of 4 staff members reviewed.

Evidence:
1. Staff #3 began employment on 8/17/2023. The fingerprint check results in the record for Staff #3 were dated 8/23/2023
2. Staff #4 began employment on 9/13/2023. The fingerprint check results in the record for Staff #4 were dated 9/22/2023.
Plan of Correction: The center will ensure background check results are obtained prior to a staff members 1st date of employment.

Based on record review, the center did not obtain a copy of the results of a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff #3 has resided outside of the state of Virginia within the past 5 years.
2. The record for Staff #3 did not contain the results of a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from the state of Maryland.
Plan of Correction: The center provided documentation that the central registry check has been requested. The sex offender registry check was completed during the inspection.
