Inspector Notes

An unannounced, on-site code compliance inspection was initiated and completed on 10/24/2024. The on-site inspection began at 8:15am and ended at 9:40am. The inspector reviewed compliance in the areas listed above. There were 37 children present with 5 staff. The inspector reviewed 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliances with applicable code sections, and violations are documented on the violation notice issued to the program.

Centers must obtain a completed national criminal background check prior to the employee's first day of employment. The completed national criminal background check in the record for Staff #2, was completed 7 weeks after Staff #2's first day of employment.
Plan of Correction: Criminal history record checks will be completed prior to employment.
Standard 22.1-289.035-B-3
The center must request a search of the central registry prior to the employee's first day of employment. Staff #2, who had been employed for two months did not have a central registry search requested.
Plan of Correction: The center requested the required background check.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #2, employed for more than 2 months, did not have documentation of requesting a central registry check from one state.
