Providers must obtain a completed national criminal background check prior to the employee's first day of employment. The fingerprint check results for Caregiver #1 were obtained 5 days after their first date of employment. The fingerprint check results for Caregiver #4 were obtained two weeks after their first date of employment. The fingerprint check results for Caregiver #5 were obtained 9 days after their first date of employment.
Plan of Correction: The center will ensure fingerprint check results are obtained before staff's first date of employment.
Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #3 resided in three states outside of Virginia in the preceding five hears. The results of the central registry for State #1 and 2 were requested and obtained more than 7 weeks after Staff #3's first date of employment. The results of the central registry for State #3 were requested more than 7 weeks after Staff #3's first date of employment and no results are in the record. The center did not obtain a criminal history record check for State #1. Staff #3 has been employed for more than two months.
Plan of Correction: The center will obtain out of state background checks within required time frames. Documentation to complete the request for State #1 was downloaded during the inspection and will be completed.
