Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on October 17, 2022, from 10:15 a.m. through 11:30 a.m. Four children and four staff records were reviewed and update.

Based on documentation review, the facility failed to obtain fingerprints for one staff member prior to beginning to work with children.
Evidence:
1. Four staff files were reviewed.
2. Staff #2 did not have documentation of obtaining fingerprints prior to beginning to work with children.
3. Staff #2 did have documentation of a fingerprint however, the correct vendor/procedures were not used.
4. The Administrator confirmed fingerprints for Staff #2 were not obtained through using the correct procedures.
