Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on 3/25/22. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children (11 points) present with three caregivers. The children were observed engaged in arts and craft activities, playing with table top toys and washing hands after activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 children's records, 2 caregiver's records, 2 household member records and emergency drill records were reviewed. There were no children with medications on site.

2Evidence:
1. Caregiver # 1's most recent TB screening was done on 9/16/19.
2. Household Member # 1's most recent TB screening was done on 9/17/19.
3. Household Member # 2's most recent TB screening was done on 9/12/19.
4. TB screenings were not renewed every two years for the above mentioned persons.
Evidence: The paint on the wall under the light switch in the bathroom used by the children was chipped and peeling.
