Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 06/23/2021. The Licensed Program Manager was contacted by telephone to initiate the inspection. The inspector emailed the Licensed Program Manager a list of items required to complete the inspection. The Inspector reviewed (1) new staff record submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined that there was (1) non-compliance with an applicable standard or law and was documented on a Violation Notice issued to the facility.

Based on review and interview, one new staff file indicated that the child day center did not obtain a copy of the results of a criminal history record information check from a state in which the individual has resided (in the preceding five years) before reporting to work.

Evidence -
1. On the date of inspection (06/23/2021), it was determined that the start date for Staff #1 was listed as 04/15/2021. It was also determined that on a sworn statement dated 04/07/2021, the State of Illinois was listed as the explanation to the question, "Have you lived outside of Virginia in the past five years?" where Staff #1 answered "Yes."
2. Based on written correspondence with a center representative, "Procurement needed a completed W-9 from IL State Police in order to write a check for them." Once that part of the process was completed, the request was sent out on 06/01/2021.
Plan of Correction: Once the out of state criminal record check for the State of Illinois has been obtained for Staff #1 it shall be reviewed by the Licensing Inspector.
