Inspector Notes
An unannounced complaint inspection was conducted January 24, 2023 and concluded May 24, 2023. The department received allegations in the area of Programs. A total of 4 child records and 2 staff records were reviewed.
The evidence gathered during the investigation determined the complaint to be not valid. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Violations
5Evidence: The record of staff #1, hired 09/01/2022, did not contain documentation of the results of an out-of-state sex offender check. The record of staff #1 contained documentation of the results of an out-of-state child abuse and neglect registry dated 11/21/2022. Staff #1 indicated living in another state on the staff's sworn disclosure statement.
Evidence: The record of staff #1, hired 09/01/2022, contained documentation of fingerprint results dated 04/21/2022.
Evidence: The record of staff #1, hired 09/01/2022, did not contain documentation of a tuberculosis screening.
Evidence: In an interview with staff #1 on 02/07/2023, he confirmed he is alone with children on the playground for approximately 45-50 minutes each day, weather permitting. The record of staff #1 did not contain documentation of CPR certification.
