Inspector Notes

An unannounced non-mandated monitoring inspection was conducted remotely and concluded on January 25, 2024. The inspector reviewed compliance in the area of administration.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard 8VAC20-780-40-C
Based on interview and email correspondence, the licensee failed to ensure to afford the superintendent or his agents the right at all reasonable times to inspect facilities.
Evidence: 1. During an inspection on 12/12/23, the center was cited for not obtaining background checks for three staff and the licensing inspector requested that within 10 days the center provide documentary evidence that the background checks had been requested.
2. The licensing inspector sent an email on January 8, 2024, requesting the documentary evidence be sent by close of business on January 9, 2024. Another email was sent on January 23, 2024, requesting the documentary evident be sent immediately.
3. As of January 25, 2024, the documentary evidence has not been received.
