Plan of Correction: The student's parents were notified of the incomplete form and they came in to complete it. Staff will begin to review paperwork upon receipt to ensure it is completed before the parent leaves.

If a key is used for a medication box, the key shall not be accessible to the children. In the Sunflower class, the key to the medication box was inserted into the locking mechanism of the medication box and it was accessible to children.
