Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection (SHSI) was initiated on 3/24/21 and concluded on 3/25/21. The owner/director was contacted by telephone to initiate the inspection. There were 40 children present with eight staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined one violation with subsidy regulations that is documented on the violation notice issued to the facility. If you have further questions about this inspection please contact Donna Liberman at 540-359-5244 or Donna.Liberman@dss.virginia.gov.

Standard 22VAC40-665-540-A
Based on review of three staff records, the center did not have documentation of a tuberculosis (TB) test/ screening for all staff at the time of employment and prior to coming into contact with children. Evidence: the record for staff #1 (date of hire: 3/8/2021) had a TB test/screening dated: 3/22/21; the record for staff #3 (date of hire: 2/23/2021) had a TB test/screening dated: 3/22/21.
Plan of Correction: Going forward will ensure staff have documentation of a negative TB test/screening prior to hire.
