Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A subsidy health and safety (SHSI) inspection was initiated on 2/4/22 and concluded on 2/4/22. There were 6 children present with 2 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with subsidy regulations and violations were documented on the violation notice issued to the facility.

Based on interview, the provider failed to insure all staff completed the department's health and safety update course every year effective 3/1/19.

Evidence:

The provider stated she had not completed the department's health and safety update course and neither had the assistant.
Plan of Correction: Both the provider and the assistant will complete the department's health and safety update course as soon as possible and will update the training every year after that.
