Inspector Notes
An unannounced monitoring inspection was conducted on-site February 28, 2023. The provider was available during the inspection. There was one child present, age 2, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 2 caregiver records, and 1 household member record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations
8Evidence: 1. The record of caregiver #2 did not contain documentation of a five year repeat fingerprint check. The last reviewed documentation of caregiver #2's fingerprint was dated 2/8/18.
Evidence: 1. The record of household member #1 contained documentation of fingerprints dated 2/26/18. Caregiver #1 confirmed the fingerprint check was not redone.
Evidence: The record of caregiver #2 did not contain documentation of the fingerprint determination letter. The provider stated she couldn't find it.
Evidence: The record of caregiver #1 did not contain documentation of current CPR certification. The most recent CPR certification available for review expired 8/22/21.
Evidence: The record of caregiver #1 did not contain documentation of current first aid certification. The most recent first aid certification available for review expired 8/22/21.
Evidence: The following was observed: 1. The swing set on the left of the property is not sturdy, had open, rusted S-hooks, and a broken and inoperable swing seat. 2. There are sharp metal pieces at the bottom of a broken pole that's supposed to be connected to double-swing. 3. There is a broken seat on the teeter swing. 4. The swing set on the right side of property has 4 S-hooks that are open enough to fit a pinky finger inside. 5. The plastic teeter totter on the ground is broken, creating sharp plastic edges. 6. The wooden playset is not a sturdy structure. 7. The provider acknowledged the broken and rusted equipment.
Evidence: 1. Caregiver #1 was observed changing a diaper. Caregiver #1 did not wash the child's hands after diapering the child. 2. Caregiver #1 stated she was not aware children's hands had to be washed after diapering.
Evidence: 1. Upon arrival of the inspection at approximately 1:10pm one child was in the care of one caregiver. An odor of a soiled diaper (bowel movement) was detected immediately upon entering the home. 2. At 1:30pm the inspector informed caregiver #1 that child's diaper needed to be changed.
