Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on October 1, 2024, as part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as a result of the last inspection related to Hanging, suffocation, and strangulation hazards, and supervision. The on-site inspection began at 12:37pm and ended at 1:10pm. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers. The inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 16, 2024. A POC submitted after this date will not appear on the public website

The family day home shall keep a written record of children in attendance each day. At the time of the inspection, attendance had not been taken since July 17, 2024.

Caregivers shall wash their hands with liquid soap and warm running water after diapering a child. Caregiver #2 didn't wash their hands after diapering a child.
