Inspector Notes
An unannounced, on-site monitoring inspection was initiated on February 4, 2025 and concluded on February 12, 2025. The on-site inspection began at 3:02 p.m. and ended at 4:08 p.m. The inspector reviewed compliance in the areas listed above. There were 8 children present and 1 caregiver; the point total was 24. The inspector reviewed 8 children?s records, and 1 household member record on-site. This inspection included document review, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 20, 2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to the child?s attendance or within 30 days after the first day of attendance. There was no documentation of a physical exam for child #2 and child #4, who had been in care for five months. There was no documentation of a physical exam for child #3, who had been in care for 11 months.

Standard 8VAC20-800-150-C
An assistant 18 years of age or older shall not be left alone with children in care for more than two hours per day. The inspector arrived at the family day home at 3:02 p.m. on February 4, 2025. There was no answer at the door, but children could be heard inside. The provider arrived at the family day home at 3:23 p.m. with no children and stated she was returning from a 1:00 p.m. doctor's appointment. Assistant caregiver #1, over the age of 18 years, was left alone with the children in care for more than two hours.

Standard 8VAC20-800-280
Poisonous materials shall be stored in an area inaccessible to children. Many poisonous materials were on the floor and other areas in the master bedroom, where children were in care, on the floor in the kitchen hall leading to the laundry area, where an infant was placed on the floor, and on the kitchen counters, all of which were accessible to the children in care.

Standard 8VAC20-800-560-A
A caregiver shall be physically present on site and provide direct care and supervision of each child at all times. Direct care and supervision of each child includes awareness of and responsibility for each child in care, including being near enough to intervene if needed. An infant (age 13 months) was placed on the floor in the laundry area by a household member and was surrounded by poisonous substances within reach, assistant caregiver #1 was in the back master bedroom with the door closed.

The provider shall ensure that a caregiver does not exceed 16 points. Assistant caregiver #1 provided care for eight children totaling 24 points for over two hours.

Medication, except for those prescriptions designated otherwise by a written physician order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children. Medications were on a dresser in the master bedroom where children were in care.

Before attendance, documentation of immunizations is to be obtained for each child. Documentation of immunizations for child #1 was not obtained until three days after the child began attending. Documentation of immunizations for child #2 was not obtained until more than two months after the child began attending. Both children were in care during the inspection.
