During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection.
Inspector Notes
An unannounced, on-site focused inspection was initiated and completed by two inspectors on April 3, 2025, as part of probation. The Department issued a Notice of Intent to Place Licensee on Probation and Require Licensee to Contact Parents, Guardians or Other Responsible Persons Regarding Health and Safety Violations on March 24, 2025, as a result of high-risk violations. The on-site inspection began at 11:04 a.m. and ended at 12:05 p.m. The inspector reviewed compliance in the areas listed above. There were 9 children present and 2 caregivers; the point total was 27. The inspector reviewed 2 children's records and 1 caregiver record on-site. This inspection included document review, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 16, 2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
The provider shall obtain documentation of a physical examination prior to the child's attendance or within 30 days after the first day of attendance. There was no physical exam record available for child #1, who had been in care for over three months.

The provider must orient assistants by the end of their first week of assuming job responsibilities. Assistant Caregiver #1 provided care for children for more than a week without receiving orientation.

Standard 8VAC20-800-540-A
Infant carrier seats, swings, strollers, feeding or activity tables, and high chairs shall be used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened. Child #1 (age 14 months) was observed unstrapped in a high chair and child #3 (age 16 months) was observed unstrapped in a bouncy seat on the floor.

Standard 8VAC20-800-590-B
An infant, toddler, or preschool child who falls asleep in a play space other than his own crib, cot, mat, or bed shall be moved promptly to his designated sleeping space if the safety or comfort of the infant, toddler, or preschool child is in question. Child #6 (age 11 months), was covered with a blanket while asleep in an infant swing. Safe sleep is defined as a baby that sleeps alone on his/her back, on a firm surface, in his/her own crib or bassinet, apart from other people, objects, toys, pillows, blankets, and smoke and substances, always during every sleep time and practiced by every caregiver. The provider was provided the Safe Sleep 365 infographic in July 2024 with this information.

A child's record shall contain all the required information. The records of children #1 and #2 is missing the address of an emergency contact person. The record of child #1 is missing the name, address, and phone number of a second person to contact in an emergency, and the work address and phone number for both parents.

Standard 8VAC20-800-600-B
Infants and toddlers shall spend no more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment. Child #1 (age 14 months) was observed confined in a high chair for 40 minutes until the licensing inspector intervened. The child was not observed eating or drinking during this timeframe.
