During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection.

Inspector Notes

The Department issued a Special Order on April 22, 2025, to Place Licensee on Probation and Require Licensee to Contact Parents, Guardians or Other Responsible Persons Regarding Health and Safety Violations, as a result of high-risk violations. An unannounced, on-site renewal inspection was initiated and concluded by two inspectors on July 16, 2025. The on-site inspection began at 10:57 a.m. and ended at 12:10 p.m. The inspectors reviewed compliance in the areas listed above. There were 10 children present and 3 caregivers; the point total was 20. The inspectors reviewed 2 children?s records, 1 household member and 2 caregivers records on-site. This inspection included document review, tour of the home, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 30, 2025. A POC submitted after this date will not appear on the public website.

A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver #1 and household member #1 was not completed and was overdue by three weeks.

Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. There were ripped and torn floor mats in the threshold leading to the kitchen and there were two bags full of trash in the kitchen accessible to children.

Poisonous materials shall be stored in an area inaccessible to children. There was hand sanitizer and Rustoleum on the porch where children enter the home and Miracle Grow in the yard, all accessible to children. There was air freshener in the living room that was accessible to the children.
