During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection.
Inspector Notes
The Department issued a Special Order on April 22, 2025, to Place Licensee on Probation and Require Licensee to Contact Parents, Guardians or Other Responsible Persons Regarding Health and Safety Violations, as a result of high-risk violations. An unannounced, on-site monitoring inspection was initiated and completed on August 11, 2025, as a part of probation. The on-site inspection began at 1:45 p.m. and ended at 5:35 p.m. There were 5 children present and 3 caregivers. This inspection included tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 4, 2025. A POC submitted after this date will not appear on the public website.
Violations
4The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 07/19/25, were not posted in a conspicuous place on the premises.
Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. Two bedrooms of the home, occupied by children in care, had floors that were covered with clothing and other items, which left no path for the children to walk without tripping hazards
Systemic Violation
Poisonous substances and materials shall be stored in an area inaccessible to children. There were spray bottles of cleaners on the kitchen counter, kitchen cleaner on a shelf and synthetic cycle oil on the desk in the back daycare room, all within reach of children.
A copy of any special order issued by the department shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations. The Special Order dated 04/22/2025 was not posted at any public entrance of the licensed premises during the inspection on 08/11//2025.
