During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection.
Inspector Notes
The Department issued a Special Order on April 22, 2025, to Place the Licensee on Probation and Require Licensee to Contact Parents, Guardians or Other Responsible Persons Regarding Health and Safety Violations, as a result of high-risk violations.
An unannounced, on-site focused inspection was initiated and concluded on October 15, 2025, as part of probation and the provisional licensure period. The on-site inspection began at 11:35 a.m. and ended at 12:10 p.m. There were 4 children present and 2 caregivers; the point total was 11. This inspection included tour of the home, interviews, and observations.
Amended 2/10/26 due to a scrivener's error.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 28, 2025. A POC submitted after this date will not appear on the public website.
Violations
4A copy of the notice of intent issued by the department shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations. The notice of intent issued, 3/24/25, has not been posted.
Systemic Violation
Poisonous substances and materials shall be stored in an area inaccessible to children. The sliding door leading to the laundry area, where bleach and several other spray cleaners were on the floor, was not locked and accessible to the children.
Medication, except for those prescriptions designated otherwise by a written physician order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children. Medications were in an unlocked upper cabinet in the kitchen.
Systemic Violation
A copy of any special order issued by the department shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations. The special order dated 4/22/25, was not posted at the public entrance of the licensed premises during the inspection on 10/15/25.
