1. The license was not posted in a conspicuous place and the most recent inspection, dated 12/10/25 was not posted on the premises.

1.The sworn statement for caregiver #2 was dated nine months after their first day of employment.

1. There were several empty grocery type plastic bags in an unlocked accessible cabinet under the sink in the bathroom used by children.

Poisonous substances and materials shall be stored in an area inaccessible to children.
1. Several packages of liquid ant bait were in an unlocked cabinet under the sink used by children and a container of disinfecting wipes were on the kitchen counter, all accessible to children. Throughout the inspection, children were observed wandering through rooms of the home in which staff were not present.
Plan of Correction: I will check the home daily for poisonous substances to be stored in an area inaccessible to children and locked in cabinet's [sic]. I will make sure cabinet in bathroom stay [sic] locked at all times.

1. The hot water tap available to children ran for approximately one minute and never reached a temperature above 57?F as measured with a digital thermometer.

A caregiver shall provide direct care and supervision of each child at all times, including being near enough to intervene if needed.
1. Caregiver #1 left child #2 (age 2 years) unattended on the diapering table.
2. Child #4 (age 19 months), climbed on a bench, pulled a push pin from a bulletin board, and began inserting the sharp end of the push pin up the child's nose. OCCHS staff had to intervene.
3. Children were observed in rooms where caregivers were not present.
Plan of Correction: I will provide direct care and supervion [sic] for each at all times. I will make sure that children are not left unattended and make sure caregivers are close enough to intervene if needed.

Children?s records shall contain all the required information.
1. The record of child #1, enrolled for over a month, did not contain the following: a) name, home address, and telephone number of each parent who has custody, b) name, address, and telephone number of each custodial parent?s place of employment, c) name, office address, and telephone number of the child?s physician, d) name, address, and telephone number of one of two designated emergency contact persons, e) information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation, f) name and policy number of the child?s medical insurance if applicable, g) chronic physical problems, pertinent developmental information, and any special accommodations needed, and h) written authorization for emergency medical care, written authorization if the child is taken off the premises of the family day home.
2. The record of child #5, enrolled for over a week, did not contain documentation of written authorization for emergency medical care.

1. Caregiver #1 used their hands and touched the exterior surface of the storage system when disposing of a diaper and wipes.
Plan of Correction: The trash can for soil [sic] disposable diaper's [sic] has proper bag's [sic] so caregiver [sic] don't [sic] need to touch surface [sic].

Before the child's first day of attendance, parents shall be provided required information in writing.
1. There was no written information for parents available for child #1, who first attended in December 2025.
2. The parents of child #5 were not provided in writing the following required information: a) holidays or other scheduled times closed, b) telephone number where a message can be left for the caregiver, c) check in and check out procedures, d) whether or not there is liability insurance, e) amount of time per week that an adult assistant or substitute provider instead of the provider is scheduled to care for the child and the name of the adult assistant or substitute provider, and f) policies for termination of care. Child #5 had been in care for over a week.
Plan of Correction: I have a contract book that i [sic] give all parent's [sic] that has holiday's [sic] and day's [sic] when daycare are [sic] closed and other information that parent's [sic] need to know about.

1. The thermometer in the freezer showed the temperature as 29?F, which is not below 0?F.
