Inspector Notes
An unannounced monitoring inspection was conducted today from 8:00am-9:20am. There were 8 children (19 points) directly supervised by the caregiver and 1 assistant caregiver. The home, 3 caregiver records, 4 children?s records, evacuation drills, and policies were inspected. Children were observed playing freely. Areas of non-compliance are identified in the Violation Notice.

7Based on record review, the provider did not obtain documentation of a physical examination within 30 days after the first day of attendance for 1 of 4 children reviewed.
Evidence:
The record for Child #1 (start date 2/26/2024) did not contain documentation of a physical examination.
Based on record review, 1 caregiver record did not contain all required documentation.
Evidence:
1. The record for Caregiver #2 did not contain the caregiver's date of hire and name, address, and telephone number of a person to be notified in an emergency.
Based on record review, the provider did not maintain written documentation of orientation for 1 caregiver.
