Inspector Notes

A subsidy health and safety (SHSI) monitoring inspection was initiated on February 8, 2022 and concluded on March 25, 2022. An unannounced complaint inspection was also conducted. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on a review of staff records, the provider did not ensure within the first 90 days of employment of service all caregivers completed Virginia Pre-Service Training for childcare staff.
Evidence: The record of staff #1 (hired 4/15/19) contained documentation of preservice training dated 4/11/20.
Plan of Correction: Ensure all training is completed in the required timeframe.

Based on a review of staff records and interview, the vendor did not ensure each staff member had current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care and include an in-person competency demonstration.
Evidence: The record of staff #1 (hired 4/15/19) contained documentation of CPR certification that expired May 2021. Administration acknowledged the staff didn't have a current CPR certification.
Plan of Correction: Ensure this training is completed prior to expiration date.

Based on a review of staff records and interview, the vendor did not ensure each staff had current certification in first aid appropriate to the ages of children in care.
Evidence: The record of staff #1 (hired 4/15/19) contained documentation of first aid that expired May 2021. Administration acknowledged the staff didn't have a current first aid certification.
Plan of Correction: Ensure this training is completed prior to expiration date.
