Inspector Notes

An on-site complaint inspection was initiated on March 22, 2022 and concluded on March 31, 2022. A complaint was received by the department regarding allegations in the areas of supervision and toileting procedures. The director was available during the inspection.

The evidence gathered during the investigation did not support the allegations of non-compliance with standards or law. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on an interview, the center failed to keep records and reports on staff and children required for two years after termination of services or separation from employment unless otherwise specified.
Evidence: During an interview with Administration it was reported that they can't find the record for staff #1.
Plan of Correction: Ensure employee records are kept for the required amount of time.
Standard 8VAC20-780-550-P
Based on a review of records on March 22, 2022, the center did not ensure that written records of children's serious and minor injuries contained all the required information.
Evidence: The incident report for child #5 did not contain documentation of the time a parent was notified of the injury and did not contain documentation of the how the parent was notified of the injury.
Plan of Correction: Ensure all staff fill out written injury records in their entirety.
