Inspector Notes

A subsidy health and safety (SHSI) monitoring inspection was initiated on-site on March 22, 2022 and concluded March 31, 2022. A licensing complaint investigation was also conducted on this date. During the course of the licensing investigation information gathered determined non-compliance with applicable subsidy standards or law and can be found on the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on staff and witness interviews, the vendor failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
Evidence: During interviews with staff it was reported that child #3 (enrolled 2/7/20) received an injury on July 14, 2021, while at the center and that injury required outside medical care. An outside witness confirmed child #3 received medical treatment and that the center was notified of such on July 14, 2021. Administration reported they were aware of the injury requiring medical attention and confirmed they did not report it to the department's representative.
Plan of Correction: Notify licensing w/in 2 business days of any injury that requires outside medical treatment.
