Inspector Notes
A monitoring inspection was conducted July 13, 2023. The inspector was on site from approximately 10:40 am until 12:00 pm. The director was not on-site during today?s inspection. There were 64 children present, ranging in ages from two years to ten years, with 11 staff supervising. Children were observed in a verity of activities including a group game of letter bingo, playing with blocks, doll house and cars, coloring pictures of sea life and making jellyfish with paper plates and streamers. School age were dancing with the interactive ?just dance? video. Later lunch was being served.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
A total of seven child records were reviewed. Six staff records were reviewed on July 17, 2023, from approximately 9:30 am until 10:55 am with the director.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

4Evidence:
The playground had multiple areas with landscaping fabric exposed. The landscaping fabric was exposed and posed a tripping hazard to children in care.
Evidence:
The slide located on the center's toddler playground did not have the required amount of resilient surfacing in the fall zone at the end of the slide shut. There must be at least six inches of resilient surfacing throughout the fall zone. The fall zone at the slide had approximately 1 inch of pea gravel. Hard compacted dirt was visible in the fall zone area.
Evidence:
Staff #6 was observed supervising 12 school-age children. Staff #6 was the only staff member assigned to supervising the children in the classroom. Staff #6 did not have current certification in CPR as of 07/13/2023.
