Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted on 3/14/2023 with the assistant director. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined a violation with subsidy regulations and was documented on the violation notice issued to the facility.

Based on review of documentation, not all staff had current certification in CPR that included an in-person competency demonstration. Evidence: Staff #1, start date 9/6/2022, CPR certification was 'exam only' and did not include the skills portion of the training.
