Inspector Notes

An unannounced monitoring inspection was conducted beginning at 1:30 p.m. and ending at 4:30 p.m. There were 60 children in care ages 6 months through 5 years old under the care and supervision of 8 staff.

Four children's records and three new staff records were reviewed. Five medications, authorizations and three allergy care plans were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Based on review of staff records, the center did not obtain documentation of the results of fingerprint background checks by the first day of employment. Evidence: Staff #1, hired 4/24/2023, national fingerprint check was dated 4/28/2023.

Based on review of diaper ointments, it was determined that not all requirements were met. Evidence: In the infant room, there was a diaper ointment not labeled with the child's name.

Based on observation and interview with staff, all prepared infant formula was not dated and labeled with the child's name.
Evidence:
1. In the infant room fridge, there was a 'arcadia brand' water bottle filled with a white liquid that was not dated or labeled with the child's name.
