Inspector Notes

An unannounced monitoring inspection was conducted with the director on March 4, 2024, beginning at 10:08 a.m. and ending at 12:20 p.m. There were 48 children, ages 3 months through 5 years old, in the direct care of 15 staff. The children were observed doing educational activities and playing with age-appropriate toys. The children were also observed eating lunch which consisted of alfredo pasta, mixed vegetables, apple slices, and milk. Five children?s records were reviewed and in compliance. Three staff records were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. Four medications and two allergy care plans were reviewed and in compliance. The menu, daily attendance, and emergency drill log were reviewed. The most recent evacuation drill was documented as 2/22/2024. The most recent shelter-in-place drill was documented as 1/8/20234. The most recent lockdown drill was documented as 2/16/2024. The most recent health inspection was dated 11/14/2023.

Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition. Evidence: The fencing in front of the center was in disrepair and was leaning. There was a gap in the fence that created an entrapment hazard.

Standard 8VAC20-780-500-B
Based on observation, the facility did not ensure the diapering surface was cleaned with soap and at least room temperature water and sanitized after each use. Evidence: After completing a diaper change, staff was observed using sanitizer to clean the diapering surface. The diapering area should be cleaned with soap and water before sanitizer can be utilized.

Based on observation, the facility did not ensure that all prepared infant formula was dated and labeled with the child's name. Evidence: In the infant classroom, there were two prepared bottles of infant formula that was not dated.
