An unannounced investigation was conducted on March 26, 2024 from 11:54 a.m. to 12:40 p.m. or a self reported incident regarding a forbidden action. Staff interviews were conducted and facility documentation was reviewed. A violations was issued in the areas forbidden action.

Based on staff interviews, it was determined that staff engaged in a forbidden action. Evidence: On April 24, 2024 staff #1 pinched a three year old child a punishment for not following directions while on the playground.
Plan of Correction: The facility contacted licensing and Child Protective Services to report the incident. The staff's employment at the facility was terminated.
