Inspector Notes

An unannounced, on-site code compliance inspection was initiated and completed on 11/18/24. The on-site inspection began at 1:05 pm and ended at 2:15 pm. The inspector reviewed compliance in the areas listed above. There were 11 children present with three staff. The inspector reviewed five children?s records and six staff records on-site. This inspection included: document review, tour of the facility, interviews,

Authorize the child day center, family day home, or family day system described in subsection A to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Staff #5, who has been employed four months, did not have an out of state background check on file.
Plan of Correction: The center will have an out of state background check completed.
