Inspector Notes

An unannounced monitoring inspection was conducted on 5/7/2024 at 12pm. There were 8 children (16 points) directly supervised by 3 caregiver(s). The home, 5 caregiver records, 5 child's records, and household member's records, evacuation drills, emergency supplies, and policies were inspected. There were no children's medications on site today. Children were observed playing freely outside. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Based on review, the provider did not ensure that each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the physician's designee, or an official of a local health department.
Evidence: reviewed 5 children's records and found that Child 3 did not have documentation of a signed immunization record.

Based on review, the provider did not ensure that children's records were maintained with the required minimum information.
Evidence: 5 children's records were reviewed:
1) Child 2 did not have documentation of liability insurance.
2) Child 3 did not have documentation of medical insurance information
3) Child 2 did not have documentation of the parent's annual review of emergency contact information. The last documented review was completed on 1/12/22.
