During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was completed on 11/12/2024, as a part of the licensure period. The on-site inspection began at 10:45 am and ended at 11:50 am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 1 caregivers; the point total was 15. The inspector reviewed 7 children?s records and (#) caregiver/household members records.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

IF VIOLATIONS ARE CITED, AND A POC HAS NOT BEEN RECEIVED, ADD THIS PARAGRAPH: Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. The license and any other documents required shall be posted in a conspicuous place on the licensed premises.

The most recent license was not posted conspicuously; the prior license that expired 6/25/23 was posted insteaed.
Plan of Correction: A copy of the most recent license has been posted, is is on easy view of the families. License dated June 20, 2023 through June 19, 2025.

Documentation shall be maintained of emergency evacuation drills.

The provider did not document an emergency evacuation drill in the month of October.
Plan of Correction: The emergency evacuation was performed on October 31, but was not recorded. Is has now been recorded.
