During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/17/2025, as a part of the licensure period. The on-site inspection began at 11:02 AM and ended at 12:15 PM. The inspector reviewed compliance in the areas listed above. There were 12 children present and 3 caregivers; the point total was 33. The inspector reviewed 4 children?s records and 1 caregiver record on-site. This inspection included: document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/29/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-540-A
Swings, feeding or activity tables, and high chairs shall be used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened.
One infant was observed in an infant swing with no safety strap fastened.
Multiple children were observed in a feeding table and high chairs without the safety strap fastened.
Plan of Correction: I will make sure every child wears a seatbelt before eating.
Standard 8VAC20-800-750-A
When topical skin products such as diaper ointments are used, the following requirements shall be met: written parent authorization shall be obtained and the product shall be labeled with the child's name.
Two diaper ointments were not labeled with the child's name.
There were no written parent authorizations available for either product.
Plan of Correction: I will have the parents sign the authorization form and label the child's name.
Standard 8VAC20-800-940-A
Temperatures shall be maintained at or below 40?F in refrigerator compartments and at or below 0?F in the freezer compartments.
The refrigerator temperature was registered as 50 degrees Fahrenheit.
The freezer temperature was registered at 10 degrees Fahrenheit.
Plan of Correction: I will make sure to keep the refrigerator at the right temperature.
